At Solace Oral Surgery, we’re committed to providing you with the best possible care. This Patient Center is designed to give you easy access to everything you need before and after your procedure, including necessary forms, pre-operative instructions, and a place to share your experience with us.
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Immaculate customer service. Both receptionist were nice, helped with directions and clarification on any questions I had. The dentist’s assistances were great as well, both had great attitudes and informative. The dentist was very friendly, welcoming, honest and straight forward.
