After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. Dr. Romero, performed a full mouth extraction of 19 teeth on Nov. 4, 2025. The day of my appointment was a little snag with a billing issue, however, Trevor his office Manager, ease my worries, he and Dr. Romero resolved the issue. My experience was FANTASTIC!!
