At Solace Oral Surgery, we’re committed to providing you with the best possible care. This Patient Center is designed to give you easy access to everything you need before and after your procedure, including necessary forms, pre-operative instructions, and a place to share your experience with us.
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

I had my wisdom teeth removed here a week ago and I have nothing but positive things to say about Solace! From scheduling the appointment, to the front desk workers, the assistants and the doctor - everything / everyone was amazing. The staff were friendly, informative and helpful. The surgery was quick and easy. I called to schedule a check up and they got me in the same day. Solace is the best and I am so happy I chose this place for my oral surgery! 10/10
