After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. Came in to get 3 wisdom teeth extracted 2 which were impacted.I was referred to by my dentist and the next day they got me in and immediately did my surgery. It was so quick and fast and I felt so comfortable. The pain is so minimal and that goes to show the amazing job they did. The staff was so amazing and helpful when it came to financial side, they did everything in their power to help me and I truly appreciated that. This goes to show they truly care about their patient and want what is best and will try anything so that they can achieve that. Would 100% recommend for anyone in need of oral surgery or care.
